Packetcable Provisioning requirements for CM and MTA BASIC.1

Good Morning,
We've migrated from a standard ISC DHCP running on Solaris UNIX to Infoblox Appliance. We've not had very good support from Infoblox and some of the Options will not directly work with Infoblox, or I haven't figured out how to get the MTA phones to work with Infoblox yet. Does anyone use InfoBlox? Do you have a sample scrubbed DHCP config from Infoblox you could post up please?

I figured it would best to start with the basics. I'm getting mixed signals on what is truly required for the CM config and the MTA config.

The modems are sent through our provisioning system to get service type and then tech assigns a phone number and mtaMacAddress.domain.com in the metaswitch. There is a host-name entry in the old config which is dynamic. Infoblox has a host-name string value option which is fixed. I know that would work if I set it with a fixed IP, but that defeats the purpose..

Is the host-name a requirement?

I also see that a log server is not defined. Is that required if so what's actually happening there with the transaction?

What are the base requirements to get the CM and MTAs working please?

So Here is what I've read I need from various sites:

CM:
Subnet Mask
Gateway Router IP
DNS Server IP
Log Server IP
DHCP Option 4: ToD server
DHCP Option 122 Sub option1 and 2

MTA:
Subnet Mask
Gateway Router IP
DNS Server IP
Log Server IP
DHCP Option 4: ToD server
DHCP Option 122 Sub option 3 and 6

Thanks I'm here to learn anything you have to offer.
